Thank you for chosing UC Irvine as your health care provider.
UC Irvine is the only National Cancer Institute (NCI) Designated Comprehensive Cancer Center in Orange County. We are committed to our mission - DISCOVER, TEACH, HEAL. Scientists and clinicians are working together to discover new ways to treat cancer and new ways to make current treatments safer and more effective. In order to conduct this research, we are seeking to collect tissue that is left over from studies that are required for your clinical care such as blood draws, bone marrow biopsies, or other biopsies. Except for giving potentially a small sample of blood, you will not be asked to undergo any procedure solely for this study.
This research study involves collection of leftover sample and storage for later research testing. It is currently not know how samples will be used at this time. Your name and any other identifying information will be removed from your sample. A code linking your name to the sample is only accesible by the study’s leader Dr. Angela Fleischman.
If you are willing to participate in this study we have available an online consent form. You can scan it below or access it at https://uci.co1.qualtrics.com/jfe/form/SV_88MLqAfHnTg5NIx. Or, alternatively you can ask your healthcare provider to give you more information and provide you with a paper consent form.
